If you are making a booking on behalf of a business or organization or have a partnership with the library, please enter the name of your organization. If you are not booking/renting on behalf of an organization or business, please enter "Not applicable".
Solicit additional information
The Library may solicit additional information from you or your organization, as applicable, to process your application. If you would like to provide information regarding self-identity as part of an equity-deserving group as part of your application, please do so below. This question is an optional and voluntary part of this application. Applications will be considered regardless of whether self-identification information is included. OPL is committed to advancing equity, diversity and inclusion in every aspect of its work, including the use of community display spaces. As such, OPL encourages members of equity-seeking groups to apply, and to include, on a voluntary basis, how their lived experience and perspectives can strengthen their application in this regard. Self-identification information is collected to allow Library staff to appropriately identify applicants from equity-deserving groups.

Personal information provided is collected under the authority of Sections 20(a) and (d) of the Public Libraries Act. Personal information is used to provide library services. The Ottawa Public Library (OPL) is subject to the privacy requirements under the Municipal Freedom of Information and Protection of Privacy Act.
